This is documented here on the boards, even under my name. There is a really neat trick to get it off. Do NOT cut it off!
Secondly, I think the knob does have some epoxy from the factory to keep if from rotating.
If you need any help with setting it up, I can help. But, if it just came loose, you just need to twist it back into place.
Lastly, the reverse lockout thing is just a little piece of plastic. It probably just came loose and fell under the carpeting. It is part #4490314.
