Believe it or not, just Meguiar's 'gold class' leather wipes (amazon link below). Picked them up at AutoZone when I was buying a couple of other things, just as a 'what the heck, why not' measure, since I thought I'd be replacing the thing anyway. (And actually saw the same pack at Target this weekend for $1.50!)
I used a couple of wipes at a time (2-4), rubbing aggressively at the most damaged/cracked parts of the leather. When the leather seemed fairly saturated with the stuff, I'd leave it alone and let it rest. Basically did this once every day or two. Started seeing results after a few days... I could kind of tell from application to application, it seemed a little less bad each time.
I didn't take any before photos unfortunately, but I can take a few now if you'd like.
