I rebuilt my suspension a couple of years ago. Here's some answers:
1. Sorry, don't remember. I never did poly bushings as I drive the car in the winter and they'd be too harsh. Also consider that poly bushings don't last as long as OEM rubber.
2. Yes, replace them, you'll probably find your old ones loose.
3. I did and glad I did. Make sure you get OEM.
4. I re-used most nuts but used lLoctite. I didn't use Koni but chose Bilsteins which came with new nuts.
5. Yes, the rubber will be getting worn and hard. You will also want new bolts that go into the subframe as these will almost be rusted through if you are anywhere there is salt. Use LOTS of penetrating oil (not WD40) and work the bolts in and out carefully...they break easy if they are rusted.
I recommend going OEM with all parts except for the shocks/strutt inserts.
