1. Quepos/Manuel Antonio Park. Crowded and touristy but very cool stuff. Wildlife, nature. Monkeys, sloths, lizards, snakes, etc. Quepos is a nice base for deep sea fishing, too.
2. Guanacaste/Nicoya Peninsula. Tamarindo is touristy but nice beaches. I stayed near Islita, a remote village, in a fabulous and pricey resort that's just about inaccessible by car. The further north and west you go, the less rain you will see.
3. I hear Monte Verde and the cloud forest is spectacular. Zip line tour!
4. Arenal volcano is a popular spot, although it's in a significant eruption right now. Good hiking. Stay away from Tabacon Hot Springs -- massive tourist trap.
Driving around Costa Rica is tricky -- their roads are notoriously bad. Right now is the rainy season, when many asphalt roads get potholed to hell. The many dirt roads, if they haven't been washed away by rains, are a chore to navigate even in an SUV. Maddeningly slow going in most cases, but especially in the wet.
I would buy the insurance -- in fact, I'll bet most rental places will force you to. No need to take chances.
Given that its rainy season, I would take buses/taxis and hop Sansa flights for longer distance travel. But I'm less adventurous than most.
