My advice: yes, buy the insurance. The roads are terrible. We rented a 5-speed Hyundai and it came with a mechanical locking device for the gearshift so the car couldn't be easily stolen. Gas mileage was fantastic because there are no pollution controls. The downside was extremely bad vehicle air pollution on the roads. There are virtually no enforced traffic laws outside of the city and driving was a hair raising experience. Gas was very expensive but was offset by the high gas mileage. We rented from Hertz at our hotel.
Gambling is very cheap in the casinos if you like that kind of action. Food in the restaurants I was in was also cheap - 3 of us had beers, appetizers, steak and the total was under $30 USD (I can't guarantee what type of animal that steak was made from though).
Watch out for the 14 year old prostitutes everywhere; you'll be able to spot them pretty easily because their 16 year old sisters will be pimping for them (no lie, it's all legal there). If you walk around in certain areas of the city (San Jose, at least) you will likely get propositioned.
You can live like a king down there due to the depressed economy. DON'T exchange your currency at the airport before you go home - I got taken that way and lost out because the exchange rate the vendors gave me was bogus (dumb American I was).
Of course it works better if you speak Spanish. I didn't but the people I was traveling with did.
Oh, and it's acceptable and expected for the natives in some areas to publically ogle women and comment on women's appearance so if you are taking your wife / girlfriend tell her to expect this and not to be insulted. It's just a cultural difference. It just depends on where you are and what type of people you run into.
All in all I found the residents to be very nice people.
