Go with whatever gets you the best deal. If you aren't going to have the second line installed (do you have any available attic or basement space where the line could go?), then the installation is just a matter of unplugging the existing receiver and plugging in the tivo. Then you'll have to call DirecTV and have the old receiver de-activated and the tivo activated, as well as activating the DVR service.
Don't worry whether or not the tivo unit comes with an access card- you can use the one from your existing receiver. I just checked eBay and saw a newly listed 80GB DirecTV tivo for a hundred bucks ($80 "buy it now" + $20 shipping) that didn't include an access card. I'm not sure whether or not Best Buy insists on adding an installation charge to any DirecTV hardware they sell- that should be one of your first questions for them.
Anyhow- good luck, and enjoy it when you finally get it up and running. Virtually everyone I meet who has one of them can't say enough good stuff about it.
