Randy Thatcher pointed out this thread to me on another messageboard.
I'm a licensed Professional Engineer. Civil by education, traffic and transportation by practice. Mike in Anchorage is almost right. Whenever you have an advance left turn arrow, the opposing volume of traffic is generally what dicates whether it's just a protected left (red signal after the arrow) or a protected/permissive left (green ball after the arrow). Most states make the determination based on a semi-complex mathematical equation involving approach (left turn) volume, opposing through traffic volume, number of lanes, etc.
Additionally, if it's a dual-left turn lane, it will always be just a protected left, because, quite simply, allowing two lanes of traffic to turn left against opposing traffic would be chaos (not to mention numerous lawsuits waiting to happen).
