Valve stems can definitely fail. It's uncommon, but that's why you get a new set every time you get new tires. If it is the stem and you want this set to last you out the summer, I'd just go to a tire shop and get a new valve stem put in. It shouldn't cost much, and the sidewall on the tire doesn't look dangerous to me, though others might disagree.
It's a lot harder to notice a modern low profile tire with low air pressure than is was with the tall profile tires you got a flat with in '84, so likely you just drove on it for a while without noticing, especially being on a rear wheel. If you've got 45k on a set of tires, you probably weren't taking turns to hard either :)
