Loose lug nuts

I have no idea how it is possible, but lug nuts tightened with an impact wrench came loose. Two or three were finger loose with the full weight of the car on the wheel. I guess all would have been loose in the air, but I can't say for sure. I heard something odd, but only thought to check the lugs when I got home. I couldn't tell it was the wheel squirming around. I would have thought it impossible but I guess one can undertighten even an impact wrench. No wonder the techs generally over tighten with the impact wrenches...better tight than sorry.
Can there be permanent damage to the threaded studs from the load? No highway speeds were involved, but maybe up to 30-40mph. I retorqued to 100 ft-lb.
