These are pretty common in the third world, but for whatever reason do seem especially popular in Latin America. No reason they can't be safe if properly installed with GFCI, but I'd say this is a pretty typical installation (as a bonus on this one, at least the electrified wiring is separate from the 'structural' white wiring). They definitely don't heat to first world standards unless you turn the water down to a trickle, but definitely take the edge off of a cold shower. "Solar" heating grids for showers are pretty popular too. They work fine in conjunction with a proper heater (uncommon) and otherwise give you cold water when you want a hot shower in the morning or at night and hot water during the middle of a sunny day when you don't really need.
