I ended up going with a small regional eyeglass chain. My recollection was that once I picked frames out they'd take a number of measurements and then have their lab make them up. At Costco, I picked out a pair and was kind of surprised to hear that those were the glasses I would get. Huh? Apparently they just put your prescription lens in and hand them to you. That's the thing I've always wondered about buying glasses on line. Peoples faces vary quite a bit. How can they have any expectation the glasses will fit? Do they just send you a "one size fits all" and hope for the best? And even when they did measure them, they still had to bend the frame slightly to fit me. That's a bit of an art and science--I've tried it myself and you can really ball them up. They expect you to do this yourself? The place I went to was very professional, and had a device that measures exactly how far apart your eyes are, etc. In addition they took measurements to determine exactly where the bifocals start on the lens. Again how could they NOT do that. Finally, they were quite a bit cheaper than Costco. So I guess it's been a bit of a learning experience. If buying on line works, consider yourself very lucky. As for Costco and Sams Club, go someplace where eye glasses are their ONLY business not a sideline.
