Flat Tire question

Daughter's car had a flat the other morning, threw on a snow tire and then took a look at the flat - a General Altimax, used for about 8k miles so lots of thread. Nothing that puncture the thread, valve stem intact and tight, it was new and yes a shop mounted the tire. So filled it up to 38 psi and it's held that amount for 2 days.
So - could be the work of a prankster as car was parked on the street, or does anyone have any suggestions as to what happened or does this happen sometimes from an incorrect mounting ?
On another subject was riding my 15 yr old Raliegh Mtn bike when suddenly heard a big pssst - flat front. Where valve stem is attached to tube a big hole, almost like the tube was shifting and putting strain on the valve stem. Rim is a cheap original Wienmann and boy oh boy is it worn and it has been out of round from several past incidents.
Do tubes fatigue or shift internally - and yes it was a cheaper thorn tube.
