The Best Buy extended warrranties can be useful on certain products - I've had them replace a PDA with a weak battery after two years (got to upgrade my Palm IIIc to an m130). CD and DVD players are also probably worth the warranty, mostly because any repair involving the laser is probably more expensive than the device itself (I've had experience with that one, though with a different retailer - I had one of the early portable CD players, and the problem may be reduced somewhat now).
When I don't want the warranty I simply tell them clearly right up front. The salesperson comes over and I say - "This is what I want. Don't offer me a warranty. I know that it's available. I know how much it costs. I'm not interested, and if you say anything about it I'll walk out and buy this elsewhere." That usually works pretty well :-)
One last note - Best Buy salespeople are NOT commissioned. They make straight salary. Best Buy does make a ridiculous profit on warranties though, so the salespeople are trained to push them hard - sort of like the folks at McDonalds who are trained to always ask "would you like fries with that."
