(1) Like Aerowhead says, your base boost is set way too high, and assuming you are still using APC this will defeat the safeguards APC can give through detecting knock etc.
(2) If you are worried about the internal state of your engine, head gasket, piston rings etc get a compression tester and check each piston - they are very cheap to buy and can immediately give a good indication of a problem if used correctly.
(3) If it were my car I think I would look at replacing my dump / blow off valve if I wre suffering your problems - it sounds very much like your one is shot and when higher levels of boost are produced it is leaking it back in to the intake. Just a thought but worth a try if you had a spare to hand.
