As RayF said, it's very unlikely that this problem is due to a hydraulic failure. The default mode of the clutch is "engaged". When the hydraulic system fails in any way you will be unable to release the clutch. You have the opposite problem and that suggests a mechanical failure of the clutch. Separation of the disc hub is a likely suspect or even a broken clutch shaft. The fact that the clutch has been releasing close to the floor also suggests a mechanical failure of some sort. Are all the bolts in the clutch cover? You'd get this with no bolts holding the clutch cover on. If the old hose was so internally corroded that the fluid could only be pumped through it but wouldn't return, sort of like a one way valve, that could cause this effect but that would be a pretty extreme example of a rotton hose. I've seen bad ones but never one that bad.
