I place the jack at the front or rear jacking points near the bumpers (on a 9-5), and the jackstands at the jack points on the two sides near the door sills. I never use anything on top of the jack, as the rubber friction surface on the top of the "hockey puck" saddle presumably provides some "holding" function. I do place a very thick piece of plywood/wood or two under the jack (wheels), just so that the jack wheels won't dig into the ground, and the jack would need to elevate less. Yes, when the jack is fully elevated, it does look dangerous, so I use the wood/plywood to give it a platform so it doesn't have to be fully elevated when changing wheels. For almost all other work that does not require wheels coming off, I use ramps . . . ramps built from multiple pieces of engineering strength wood that would be sufficient to hold up a mine shaft or a house. I'm too safty conscience to be using even off-the-shelf ramps for fear of slipping or deformation.
