Sometimes the failure mode is worse than your contingency plan. I've seen some pretty amazing damage to test cells caused by internal combustion engines. Luckily no one was injured in those blasts.
I remember one time when we were renovating an engine test cell that used Extra-Heavy pipe laid horizontally in the soil beneath the cell floor to route the engine exhaust out of the chamber. Even though the pipe was under negative pressure to move the exhaust faster, the pipe got so hot that it sagged under its own weight, changing from a circular cross section to something resembling a hollow bomerang shape, and the sand surrounding the pipe phase changed from silica crystals to glass!
