Was it a big one 2.5 ton, or the little 1 ton one? The lifting plate is much larger on the bigger jacks.
I've been under floor jacks for about 13 years now, and I've never had a car fall off of one. I make sure the car has the e-brake on to prevent rolling, or put something behind the tires.
I always use the big jacks, and I only use jack stands when the jack is at maximum lift. Otherwise the jackstands just get in the way of what I'm trying to get to.
