In my case it was on the outer race as well.
None of the balls were severely damaged. The vibration that I felt was most likely due to the gap in unevenly spaced balls rather damaged balls.
I my prior experience with my Datsun I had 10 times more damage on the balls with yet intact spacer. So maybe the spacer fails when the bearing is on its way out anyway. But I still think it was the spacer that failed first. It is likely to be very hard to find out for sure.
P.S. I own vw golf with 250,000 miles plus - all original bearings, cv joints, etc.
