Tim is right, the need for brake pad replacement is based upon amount of wear....not mileage. There is no such thing as a "standard" for replacing pads and rotors....no more than there is for the shoes you're wearing.
As an example, my 2000 9-5 Aero went 100K miles before needing the first set of front pads and rotors; the rears went 200K miles. All of my driving was Interstate at that time. Now that I'm retired and doing a mix of local and highway driving I'm expecting to get 30K - 40K miles before pad replacemet is required.
Get used to inspecting pad wear yourself. 4mm is a little more than 1/8". 1" is 24mm....so 4mm is 1/6" a little more than 1/8".
Replacement of the rotors is also based upon wear, not mileage. The challenge here is to research the thickness of the rotor when new and measure the remaining thickness. If the amount remaining is a lot less than the amount you've already used....replace the rotors.
On the pre 2006-2007 replacement of the rotors with the pads was common.
