probably sloppy work by the shop, some manuals have a throw rod and a locating rod, reminds me of my 1991 Taurus SHO with 12k warranty and a little beyond that mileage same thing happened to me, as it turned the factory had stripped the bolt hole for the locating rod, put some kind of adhesive in there, sticker on the car said "built with pride in Atlanta, GA" but it was anything but a well built auto, despite having a nice engine.
