If one of the cams is off a cog, you could definitely get lumpy idle, and you'd also end up with your power band shifted - a lumpy idle will generally mean bad low end and improved high end...
Do you have a reason to believe the cogs may not be aligned correctly? Something would have to go pretty badly for the timing gear to slip. That's very abnormal on a Saab.
