Both pumps run all the time. The pre-pump grabs the fuel low down and brings it up into a bowl-like area the main pump pumps from. When the tank is at least half full, the bowl area (not really, but close enough) has fuel in it and the main pump can handle it. The pre-pump still runs, but it's just sloshing fuel around.

If the pre-pump and main pumps are both replaceable separately. eEuroparts lists the pre-pump at $45. The main pump is $181. You don't need to replace both of them. The only question is - which one is bad? If the pump is just making noise, it's hard to tell which pump is bad, if it is a bad pump at all. My fuel pump troubleshooting notes show that the noise can be from parts getting loose. Yes, really loud noises can be the pump. But frankly, until one quits, it's hard to tell which it is. If the pre-pump dies, the car will quit at less than half a tank. If the main pump quits, the car quits at any fuel level. Of course, a dead car is no fun. And if it quits at less than a half tank or so, which pump is it? Adding fuel would tell you, but again, how do you add four or five gallons of gas to a car stuck on the side of the road?

Replacing the fuel pump isn't a major task - getting the fuel barbs out takes some care, and it really helps to build a pump ring removal tool (check over on the 9000 board). With the pump assembly out, you can replace the individual pumps pretty easily.

What would I do if I were you? I'd by a pre-pump, and pull the assembly. If you find something was loose, you can bolt it back on tight, and worst case, you've got a spare pre-pump. If nothing looks wrong, replace the pre-pump and hope that was the issue. If you stick it back in and it sounds terrible, then you're looking at the (main) pump motor. But I wouldn't do nothing - the pump dies, and the car stops. Not fun.
