A floor jack, a piece of wood, and the old slave works pretty well. Put the wood on the jack, the new pressure plate on the wood, the slave on the pressure plate, and the car on the slave. Then jack the car up, slowly. The pressure plate will give pretty quickly, then slip the spacer tool in.
Like cmyles said, the bolt for the input shaft is 8x1.25. A big washer is useful. Put the washer on the bolt, thread the bolt into the shaft, then use an open-end wrench to lever the bolt/washer against the transmission. A normal-length 19mm wrench works great - a quick rap with your hand and it'll pop out.
Notnoel's tip about rotating the whole assembly out together works great for me. Trying to get the parts out piecemeal is a mess. But with only a small amount of noodling they will all rotate out together wonderfully.
Unless you have a specific reason to, I wouldn't replace the rear seal. I think I've done that job twice in my life. They don't leak that often. I *would* plan on replacing the input shaft seal on the transmission, but that seal is small enough you can largely just install it by hand. Consider the pilot bearing on the flywheel, too - it's cheap and easy.
