...they're both mounted on the crankshaft. I didn't replace the idler or gears on the actual balance shafts. I actually also replaced the camshaft sprockets, but only because I am extremely preventative-minded. They really shouldn't have to be done, in most cases, and with your mileage, they should be fine.
Also add tensioners to your parts list -- one each for the balance shaft chain and timing chain.
I had bad luck fitting the timing cover in below the head gasket and ended up doing my head gasket as well. Really not that hard, and afterward, I had terrific peace of mind knowing that the head gasket and head gasket/timing cover relationship were okay.
