The flywheel step height is important here. That is the distance between the high outer rim on the flywheel and the lower center part where the disc contacts the flywheel. The specification is 0.656" plus or minus .005".
I do not think this is your problem but it is useful information.
If the holes in the bell housing where the vertical shaft rides are worn, this could cause your problem. Likewise if the clutch fork is bent.
Some of your description is hard to follow. Shimming the slave cylinder would seem to only move the slave toward the back of the car, aggravating your problem of failure to disengage. ????
And I don't quite understand that when bolting up the pressure plate, it is still 1/4" from the flywheel?
Could you have the disc in backwards?
