Haven't been down since they made the islands a park, but I believe there are boats trips out to some of the islands. You can easily kill a day. Bring a lunch, sun screen, a camera and enjoy seeing the city from the water and the islands themselves. Used to sail out there every weekend before it was a park. Boston Harbor is one of the nicest city harbors in the U.S. and worth a look. Then come back to the city and eat Italian food in the North End. Anyplace is good.
Take your car and get OUT of the city. Don't head south to Cape Cod as it is traffic hell, but go north up the coast. Marblehead (30-45 mins), Gloucester/Rockport (about and hour) are all nice. Further up is Newburyport and Plum Island (also about an hour). Great restaurants in all those towns. Distances are short, too.
Just watch out for Massachusetts drivers. Turn signals are viewed as optional, as are stop signs and red lights. Tailgating is the state sport. Road rage can be bad, so stay off the major roads at rush hour.
If you have more time, head further north to the NH coast or into Maine. You can be in Portland ME in a couple of hours from Boston. Just take I-95 north.
Or 2.5 hours up I-93 will put you in the White Mountains in NH. Some great drives up there, great scenery.
