Carfax did it to me not long ago, my Suv was in the body shop for a paint job, and Carfax put down frame damage. It took me almost a month to fix the report.
Another example, I was looking at a 2003 Toyota with 45K mileage, Carfax showed in year 2001, the same toyota already has 50K on it. I send Carfax an email saying that, that Toyota was not even made back in year 2001, how in the world you get 50k mileage on it.
