The problem may have been that the wheel was NOT removed, and that the reason the wheel fell off (sheared the bolts) was becuase the accident damaged the bolts.
They may or may not have removed the wheels to replace the bumper and fender- might depend on easier access to the inside bolts.
If left loose, the wheel would have first developed a nasty wobble, but why would the bolts be sheared? On a RF hit hard enough to require new bumper and fender, the wheel would have sttod a good chance of being hit.
