A lot of people seem to quote the donut spare speed rating like the gospel, and how evil and irresponsible anyone who goes over it is, but it's actually strictly for liability rather than the safety of the tire itself. Same for the distance rating. You can imagine how emergency handling could be severely hampered at speed with one tire being so much smaller, and the manufacturers don't want to be held liable. A few years ago one of the tire companies did Car & Driver's One Lap of America event on four donuts, which I think is a pretty good example of how they won't instantly blow up the minute you hit 60 mph.
My guess for Gene's is under inflation or a manufacturing defect, perhaps combined with age, rather than a direct breaking down with age. 20 years on an exterior wheel is a big difference than 20 years essentially in the trunk in terms of rubber breakdown, but I guess that is getting a bit old.
