There are probably many different aspects and dimensions to this beyond the diagnostic result. From my years of "endocrinology is my hobby"
Males can suffer brain fog from a few different reasons. This can be subtile or profound. There are probably some serious problems when you have these in patients with dementia.
Subclinical hypothyroidism can have major effects for many males. The labs for TSH, T3, T3, fT3 and fT4 can all be within normal range. These ranges do not imply any measure of good heath be been in range. However, many doctors have that all wrong. TSH should be near 1. The others should be near mid range. fT3 and fT4 labs are preferred as T3 and T4 include bound hormones that are not bio-available. Ultimately, fT3 regulates body temperature. If waking oral body temperature is at 97.7F or lower or if one cannot get to 98.6 during the mid afternoon, there is a functional thyroid problem. With many not now using iodized salt, I think that most cases of subclincal hypothyroidism is caused by iodine deficiency. At this point I could write pages more. Low thyroid function leads to low mental energy, brain fog, lost libido, fat gain, lethargy, dry skin/hair and brittle nails. Sea salt is causing a lot of problem.
Steroid hormones: During puberty, testosterone causes brain patterns to change. Areas for aggression/combativeness/competitiveness increase and areas for nurture/compassion are substantially pruned. With age, when testosterone (T) levels drop, estrogens (E) increase and the ratio of T:E becomes adverse; brain patterns start to drift towards a state that the virilized brain [hardware] cannot support. The result can often be described as brain fog. People notice that a guy simply is not himself any more and dismiss this as aging. However, re-balancing hormones can reverse such problems. The problems also include short tempered, confused, noise intolerance, easily startled, low libido, ED, passivity and apathy. Physically: skin tone degradation, lost muscle mass, lost leg hair, fat gain can all be corrected. Estrogens need to be balanced by T to prevent cardio vascular disease processes. Excess estrogens cause brain function problems even in males with high testosterone. Could write a book ...
I so often see guys with testosterone problems and thyroid problems together. This can really cause quality of life problems for guys who have healthy brains. The aging brain becomes more vulnerable to hormone imbalances. There are aspects of care that are ignored.
No... I am an Engineer, I spend hours every day doing this.
