If by "insurance" you mean health insurance, there's a law called COBRA that requires employers to give you the option of continuing on their plan for a certain amount of time after you leave... for a price of course. There's some good info here: http://www.dol.gov/ebsa/faqs/faq-consumer-cobra.html
As far as 401K, some employers as a courtesy will allow you to keep your $ in their system for a while, although you might have to cover management fees. But in general you'd roll the $ either into the plan of your new employer, or to some sort of personal account (like an IRA). That's something you'd want to talk to an investment adviser about.
