and came off. As the last one came loose it could really rip stuff apart coming off at 50 mph or so.
It could be more, like the axle bolt coming off, but I'd bet on the lugs not being tightened after a tire swap or rotation.
I once had a tire guy at a Sears forget to tighten ALL the lugs on the whole car. They were all on just past finger tight and came loose. Didn't loose a wheel, but it came close.
