I see exactly what the problem is. The bushing there is installed incorrectly.
Basically that bushing fits into a sleeve in the alternator mounting bracket. It is supposed to fit with that rubber collar (the wider part) INSIDE the bracket, not on the outside. So... you press the bushing in from the inside of the bracket (ie so that the alternator contacts that bushing flared area).
So... yours was incorrectly installed on the outside. Which means that even though you got the bolt/nut tight, there is slop inside where the alternator is mounted. And the nut will always work loose.
There is another bushing exactly like this bushing on the other side, where that nut (that fell out) goes. I'd bet that this one is installed backwards too.. and the net difference of those 2 bushings in backwards means that it looks like the bolt isn't long enough.
