Now that you replaced the vacuum hoses, you have eliminated a lot of the variables and can start froma good baseline. See if Limp Home Mode ever occurs while cold - I have some doubt that it will. If it doesn't occur when cold, it points to the safety valve failing when hot, a common failure mode.
I'm getting sick of nice cars getting junked under this program: a "report" on the TV news showed a lineup of the cars traded in at a local dealer, including a Mercedes SL convertible in what appeared to be nice cosmetic shape. One of my coworkers traded in his father's pristine Chevy Caprice with 85,000 miles for an Odyssey van (the gets 2 MPG better mileage, but qualifies for the program since it is a different class of vehicle!).
