If we are talking just email (not email enclosures) as long as you have scripting disabled and keep your system regularly patched risk isn't too great. Email clients should be configured with high security settings and should never be allowed to run any type of scripting. One good reason not to open spam while online and keeping auto-preview off is to avoid setting off a web bug that could result in your email being marked valid and begetting more spam. Regardless the best bet is always just to delete.
Enclosures are another story. Opening these are clearly have potential high risk.
In addition, to the above practices run some sort of virus scanner, spam filter, and firewall. And by all means KEEP your SYSTEM PATCHED. Your most likely to be hit by a known but exploit and that is silly when there is a fix available.
