OT: What do you consider to be the top auto blunders?

OK, just thought I'd throw this one out for discussion. Aside from the obvious ones such as the Edsel and the Aztek, what do you consider to be the top automotive mistakes? Here is a recent one to get the ball rolling...Mazda invests huge $$ in the Mazda6 and tells everyone it is a sports sedan, then ships only 4-bangers to the U.S. (very few 6 cylinder models were originally shipped). Massive amounts of lost sales were experienced.
Any others you care to share?
