Get access by any means, assess responsiveness, spontaneous respiration and pulse. If easily extracted at that point, slide them onto a backboard, begin resuscitation if needed. I once talked to an EMT who intubated a non-breathing occupant while they were sitting up, prior to extraction. If there is a fire, speed is of the essence, and you extract even if a backboard is not available, provided you can do so without yourself getting killed.
