yeah, anything can happen, can't it... I mean, even driving a car, people have heart attacks and drive off the road suddenly...
bike helmets are a difficult thing in the sense that they need to balance safety and also performance issues (weight, cooling). There have been various designs over the years and the early ones had a much more substantial shell... then there were the ones with no shell at all (yikes! would break apart easily if hitting anything sharp!), then the thin covering shell taped on, and now pretty much everything is the 2 part injection... I would think those are pretty strong at least for the first impact - the plastic is thin, but because it is adhered everywhere it is reinforced... so from a performance cycling perspective, those are pretty good... the heavy hardshell multisport ones (like skateboarders use, or Nutcase) are probably better for (slow) commuters, as they can withstand multiple impacts (they are designed for this)... they're heavy and hot though...
Outside of commuting (which is dangerous), the number of serious injuries per rider is very low, at least in my observation. It's actually a pretty safe sport. Much more likely to get brain injuries (ie concussions) in contact sports like hockey, football, or boxing... and doing pretty much any of these sports is far safer than sitting idle and not exercising at all!
