I've had three rotator cuff injuries and major spinal fusion (11 vertebrae) and so have spent a fair amount of time in an MRI tube and seen a few MRI reports. I think the gist of this report is that it could be either/or on the bright spot, but favors the possibility of high grade tendinosis over the partial tear because you have demonstrable tendinosis on other areas. If that leads to some cortisone injections and physical therapy, give that a run and see if surgery can be avoided.
