Hello,
I have seen the gear come loose several times. I have never seen the bolt fail. Usually It happens when doing a head gasket and not getting the gear keyway completely centered on the camshaft when tightening it it damages the keyway and allows the gear to spin. We had a tech when I was at the dealer that did it 3 times. And I have seen a few others.Yes it damaged the valves in all the ones I saw. Good Luck, Bernie
