The idler pulley is indeed smooth. You can have a pulley, go south, over a long period of time (like mine did) or go very quickly.
I can't imagine the pulley being the culprit of bad milage to that extent. Your belt would have given up the ghost long before that.
I would check the bearings of the accessories to see if that is the cause of your issues.
