I'll admit to having done that at one time. My excuse is that the starter motor in my car only worked occasionally, and I didn't want to have to go through the trouble of push-starting it (an '83 900T that was over 10 years old and 200k+ miles) in a busy gas station.
In the five times or so that I filled up in this manner before having the starter replaced, only one person mentioned it to me, and she was complaining about the exhaust fumes. When I told her that the starter was broken, she expressed a crestfallen "but I always have everyone do things MY way" look, and walked back to her car. My tank was full within 10 seconds, anyways.
As for safety- if a running car engine was a true risk, wouldn't we be forced to shut off the motor before approaching and after leaving the pumps?
