Dead Goldfish

I bought a small 10 gal gold fish tank for my toddler. Per the instructions, we added water conditioner that came with the kit. I used tap water. About a month goes by and my wife thinks I need to clean the tank. So I put the 5 fish in a bowl and dump the water out. I fill the tank back up with water from a garden hose. It was a cold day. Before putting the fish back in, my wife says the water is too cold so she puts some hot water in it. Then she puts the fish back in. Well, she thinks the water is too warm now and proceeds to empty some water and put in some cooler water. Then I see her putting Ice cubes in the tank. Note the fish are in the tank at this time. The water doesn't seem that cold, but within 10-15 minutes the fish are all at the bottom. Well they all died. Now I'm wondering did they die because of the temperature change or was it because I didn't add water conditioner?
