A 1941 house with upgraded electrical may or may not be up to code.
If the city required an electrical permit and if one was issued, then the electrical would have been upgraded to which ever National Electric Code was in effect at the time. From what you are saying, it sound like the electrical was upgraded at a time when grounded outlets were not a requirement. An older installation such as this would be "grandfathered" meeting city code but not necessarily the latest NEC requirements.
I do not see why use of a 3 to 2 prong adapter would be a code violation. Place a call to one of the city electrical inspectors.
As to "it" being safe? That depends on the condition of the appliance and the location.
Are there any GFCI's in the house? If there is a GFCI in the room, the 2-wire receptacle may already be protected. If that is the case, push the test button on the GFIC to trip it and check to see if you lost power to the refrigerator receptacle. If that is not the case and you want to play it safe, have the receptacle replaced with a GFIC and plug the refigerator into that.
