Some of your arguments don't necessarily imply cause and effect but possibly correlation. #2 might be due to the fact that the academic institutions hire people that are less accident-prone. #3 is just based on one study and that does not constitute proof. And I would argue that the NEJM may be biased. #5 shows that they can find other ways to reduce healthcare costs, perhaps lowering doctor salaries??? It's not as if they can't quickly reference other experts or computers or books. Lots of medical decisions are based on a decision tree of some sort, aren't they?
To me, it's all unnecessary hazing. You can learn only so much and you don't have to be tired to learn how to do the right thing. Police and firemen learn without being deprived of sleep and they seem to perform well under pressure. Why should any doctor work to the point of being tired even after training? If they rotated shifts more, it wouldn't be an issue.
If anything, doctors should know that sleep is necessary to healthy functioning and for proper cognitive functioning. (Now they're finding that sleep helps with learning.) Why should we support training for medical students that is harmful to the students? How can something that is bad for the students be good for their patients?
