First of all I'd like to thank everyone for their good ideas. This is a particularly brutal winter here in "Chiberia", and ice dams are not a regular event on my roof.
As stated previously, the attic has something like a foot of loose fiberglass insulation, there is a working ridge vent on the peak, and there are four large gable vents. The soffits may or may not have insulation in them, but because of the "cathedral" type roof configuration, they are completely outboard of the house and may not be getting heated internally.
In any case, the roof seems to be a normally "cold" roof, which is born out by the heaps of snow on top of it - most of my neighbors seem to have much less snow, despite a flatter pitch. The warm weather started a couple of days ago, which started the melting, etc. It's warmed up considerably the last couple of days, to the point that there is no more dripping in the house even though there is still some ice on the roof (must extend less than the length of the ice barrier) and it's raining outside.

Yes, that's kind of where I'm leaning at this point. We've lived here for 13 years and never had ice dams to this extent before. I'll probably install some heat wires over the summer, at a minimum in the gutters and downspouts - have to do a power calculation after I measure how much length I need. It's hard to get a snow rake where I need it to go, as the porch is 11' wide and 9' tall - just the hypotenuse of this is 14.2', plus the base of the rake has to get up to the second floor. I'm going to have to figure out how to get something long enough, or stand on a stepladder near the house when raking!
