The short answer is that the common Otto cycle (four stroke) internal combustion engine produces power most efficiently within a narrow RPM band. If you added higher gear ratios and dropped your engine RPMs accordingly your engine will struggle and waste fuel. You'd also find that you might be able to cruise on perfect, flat, smooth, level pavement OK but you'll have to downshift for even the tiniest hills or other impediments. You'd be doing a lot more shifting than you want to. Racing engines are built (or modified) to have high RPM power bands and won't run well at low RPMs. That's one reason race cars and motorcycles are "gunned" or "revved" at the start line while waiting for the flag to drop. The engines are trying to die at idle RPMs and gunning them prevents them from dying. You can design an engine to have it's power band high or low and some industrial engines are made to produce the most power at very low RPMs but needless to say don't do well at high RPMs and often have governors to prevent accidental over-revving and the resultant destruction.
Modern technology is expanding the effective power band of these engines and making cars that can make use of very wide gear ratios is a development that's on the horizon. Wow! Good thing that was the "Short answer"!!
