HI,
The O-ring problem was known for at least a year prior to the explosion. The engineers were trying to get NASA to stop launches so the o-ring could be re-designed (BTW, the redsign was rather simple, double the o-rings). The rockets from the launch prior to the disaster actually came back with all the seals burnt out.
The worst thing about the day of the disaster was that the launch environment was out of spec. The night before the launch, the low was 32F and the min spec was 35F. The spec was set specifically for the o-rings. Because of the high profile launch, things were forced on schedule and, rumor has it, that the launch order came from the President. The Vice Pres is the appointed NASA chief, normally.
