My son used to work as a claims adjustor for State Farm and, when I hit a deer not too long ago, he helped me through the process. So this is second hand information based on his work experience.
Generally, a car will be totaled when damage hits 60-70% of value. This will vary depending on exactly what kind of damage occurred. For example, a car with deployed airbags will usually be totalled at a lower percentage.
Other considerations factor in as well. For example, insuance companies will be totalling almost everything in the hurricane, whether it truly needs to be totalled or not, because of the volume of claims they're dealin with. For another example, a long-time, loyal customer can get his agent to work with the claims adjustors, convincing them that it's in the company's best interest to total this car even though it doesn't really fit the criteria.
As for the dealer not knowing it had a salvage title, I'd say they are either incompetent or dishonest. I cannot imagine a dealership taking a trade without running the title before hand. Even if they did not, they would know it was a salvage title as soon as the title was handed to them. Possible, I guess, but it doesn't seem very likely to me. Mike Lynch can probably address this better than I can, but I'm skeptical.
